How Much Does TRT Cost In Arizona?
Real numbers for insurance, in-person clinics and online care, with the sources. Then our price, with the one exclusion in writing.
The testosterone is the cheap part. A 10 mL vial of generic testosterone cypionate lists at roughly $36 to $87 cash1, so what TRT really costs in Arizona comes down to how visits, labs and monitoring get billed. At AZTRT it's $149 a month in 90-day terms, with your initial lab panel, provider visits, medication, supplies and shipping included. Follow-up labs are billed separately.
Three ways to pay for TRT
Arizona men usually end up in one of three models. Each one bills differently, and the sticker price rarely tells you the full number.
| Model | What you pay for | Watch for |
|---|---|---|
| Insurance primary care or urology | Copays or coinsurance, your deductible, lab bills, a pharmacy copay | Deductible first, prior authorization, and two low morning tests before coverage starts |
| In-person cash clinic | A membership or per-visit fee, labs, medication, sometimes in-office injections | Membership on top of medication, price only given on a sales call, standing appointments |
| Online clinic | A monthly program fee; labs and medication may or may not be in it | What is actually included, separate lab fees, and whether the prescriber is licensed in Arizona |
| AZTRT | $149 a month, 90-day terms | Follow-up labs are billed separately. That's the exclusion. |
None of these is automatically cheaper. Insurance can be the lowest-cost route if your plan covers TRT and you've already met your deductible. A cash price can come out lower if you haven't. The only way to know is to price your own situation, piece by piece.
What the pieces cost on their own
Here's what each part of TRT costs when you buy it by itself in Arizona, using prices you can check.
The medication
Generic testosterone cypionate 200 mg/mL lists at about $36 to $87 for a 10 mL vial and about $14 for a 1 mL vial at cash prices1. How long a vial lasts depends on the dose you're prescribed. Syringes, needles and alcohol pads come on top.
The labs
You can't start TRT without bloodwork. Guidelines call for a fasting, early-morning total testosterone, repeated to confirm a low result2. So diagnosis can mean two draws, not one.
Sonora Quest posts self-order prices for Arizona. On its July 2026 list3:
| Test (Sonora Quest self-order) | Posted price |
|---|---|
| Testosterone, total & free (adult males) | $98 |
| CBC + CMP | $43 |
| PSA | $44 |
| Estradiol | $66 |
| A basic baseline panel, bought this way | about $251 |
Prices for tests a provider orders, and what your insurer pays, will be different. At AZTRT your initial panel (total and free testosterone, SHBG, estradiol, CBC, CMP and PSA) is included in the $149.
The visits
What a provider visit costs varies too much across Arizona to quote one honest number. Insurance visits run through your copay and deductible. Cash clinics usually fold visits into a membership. Ask whether follow-ups are billed per appointment.
The monitoring
This is the part people forget to price. Guidelines call for a set monitoring plan once you start: testosterone levels, hematocrit and prostate checks, especially in the first year2. Every model has this cost. The question is who bills it and how.
TRT with insurance in Arizona
Insurance can cover TRT. Two things decide what you actually pay.
Your deductible. Among workers with a general deductible, the average for single coverage was $1,886 in 20254. Until you hit it, you're usually paying the plan's rate for visits and labs yourself.
Prior authorization. Many plans want proof before they pay for testosterone. UnitedHealthcare's 2026 commercial policy, for example, asks for two pre-treatment total testosterone results below 300 ng/dL (or below the lab's range), drawn at separate times5. Other plans set their own rules. Call yours before you book.
AZTRT is cash-pay only. Nothing is filed with your insurer. Most patients can use an HSA or FSA card; check with your plan administrator. More on that in does insurance cover TRT in Arizona.
In-person cash clinics
Phoenix, Scottsdale and the East Valley have plenty of storefront clinics. Their prices are hard to compare because many don't publish them. Before you sign anything, get straight answers to these:
- Is there a membership fee on top of the medication?
- Are follow-up visits included, or billed one by one?
- Are labs included? Which ones, and how often?
- Do I have to come in for injections? If so, price the drive and the time off work, too.
- Who is the prescribing provider, and can I verify the license?

Online TRT clinics
Online programs usually charge one monthly fee. What that fee covers varies a lot. Check three things: whether labs and medication are inside the price, whether the provider is licensed in Arizona, and whether you get a live video visit or just a questionnaire. Our guide to online vs. in-person TRT in Arizona covers the rules.

Price is one number. Ask about month four.
A cheap first month means nothing if nobody rechecks your labs or adjusts your dose. TRT is long-term care. Compare what each option costs once you're past the start, including follow-up labs and visits, because that's what you'll actually be paying.
